CE is a 23-year-old Hispanic woman who came to the hospital on April 12 at 8:00 pm because she was having contractions less than ten minutes apart in due to pregnancy, reported her pain in 8 out of 10 patients vital signs were measured on admission, her blood pressure was elevated at 132/84, patient clearing was at 50% on admission CE she is a gravida 1, para 1 (D:1/P:1) , her last menstrual cycle occurred on 5/7/13. When CE arrived at the hospital, her bottom height was 39 cm and her estimated due date was 4/12/2014. During the pregnancy EC had no complications and the fetus did not present any anomalies, she reported taking prenatal vitamins and a calcium supplement. CE had an NSVD (normal spontaneous vaginal delivery) at 9.38am on April 14 in a healthy baby girl. CE was in labor for 7 hours. The patient has an intact perineum; the placenta was delivered spontaneously and remained intact, after birth both mother and baby remained in stable conditions. History of current disease: 40 weeks gestation, full term baby, no complications, came to hospital due to contractions and disappearance of the cervix. Admission Diagnosis: CE came to the hospital because she had contractions that did not stop after changing positions, occurred at regular intervals, and steadily increased in strength, which occurred after the onset of labor. Physical assessment (on admission)-Restorative: RR was 20, labor due to pain, evident bilaterally-Cardiovascular: BP 132/84, no history of HNTGI: No bowel sounds present, patient was maintained NPONeurological: Pt alert and oriented x4.
