For starters, what is a disaster? Well, a disaster is a natural or man-made hazard that results in an event of substantial magnitude that causes significant physical damage or destruction, loss of life, or drastic changes in the environment. Therefore, an engineering disaster is a disaster caused by a mistake made by an engineer in producing the product or service. Making a mistake is part of being human. Unfortunately for engineers, due to the importance of their work in creating products to serve humanity, their mistakes usually lead to disaster. On April 26, 1986, a technical disaster occurred in Pipryat, Ukraine, then part of the Soviet Union. The disaster struck the Chernobyl nuclear reactor which exploded, releasing at least 5% of the radioactive nucleus into the atmosphere. This was the result of a faulty reactor design that was operated by inadequately trained personnel. So what happened that day to cause this disaster?
