Virginia Beach Health and Rehabilitation Center, located near the ocean, caters primarily to older adults, although there are patients who may be in their twenties . Many patients remain in the facility for the rest of their lives. However, some patients are rehabilitated until they recover or can move to another location, such as a home facility. Cindy Halonski, one of the recreation therapists, gives each patient who arrives a welcome page. This document lists information such as the different rooms, such as the courtyards, the different activities available, where the television is located and lets patients know that there is also a beauty salon on site. While this welcome sheet is not mandatory, it introduces what is available up front and the patient can remember it by looking at the page. All patients must undergo an oral history interview within a certain period of time (the current one must be done by December). The aim, according to the document, is to get to know the patient better. It passes through different aspects of an individual's life. The interview paper is divided into three categories: Childhood, Adulthood and Interpretation of life which are then further divided. Oral history is another way to build a relationship with a patient and find out more about them than a CTRS can get from a medical record or evaluation. Due to funding and patient health insurance coverage, all assessments are provided by MDS3. MDS3, which was previously MDS2, is the Medicare documentation. I think a van would be useful because it opens the door to other activities that cannot be provided at the facility. For some of the healthiest, long-term patients, it may be the first time in a long time that they can see beyond the grounds of the building. It can lead to new experiences and be as therapeutic as working within the centre. I know that ratings cannot be changed due to the MDS, however I believe the Monthly Activity Participation Record can be changed. I think this particular part could be redone to make the paper easier to understand. Especially considering that the sheet itself is quite difficult to read and that some tasks had to be handwritten. However I thought having a welcome flyer was a wonderful idea and I hope they continue to use it in the future.
